Registration Information &

Health Management Forms

 All programs are currently full for the school year 2011/12, however, waitlists are available.

Registration opens the first week in April for returning families and the second week in April for new families. At that time we register for the upcoming summer program and for the following school year. All registration is on-site at a first-come basis. A wait list is created when we reach capacity. To check for available space during the year, please contact us by phone or email.

Registration Forms

The following three forms and payment will be needed prior to your child’s first day. These are downloadable forms for you to print and fill in.

1.  Registration Form 2012 (only one is needed for BOTH summer and fall)

2.  Enrollment Contract 2012

3.  Immunization (State of WA requires both front and back pages of the form)

4. $20.00 registration fee and one half of the first month’s tuition. One third of the first month's tuition is due for Pre K. (non-refundable)

Medication - Allergy - Asthma Management

If your child has known allergies, asthma or will need to be given medication, the following forms are required. Please bring these in along with EpiPins, inhalers, or other medication your child requires. Be sure and review our Family Handbook for complete information on our Medication Policy.

1. Medication Authorization Form This form is for any medication, aside from sun screen or hand sanitizer, you require us to give your child. This includes medication we keep on hand such as inhalers or EpiPins or daily medication. For occasional day-time medication delivery, bring this form with the medication.

2. My Asthma Plan This form lists three levels of severity for us to watch for if your child has asthma and the course of action we need to take. 

3. Health Care Provider's Allergy or Intolerance Report For known allergies or intolerance to foods, etc. you will need your child's doctor to assist in completing the report.

4. Child Care Emergency Plan for Allergic Reactions Children with sever reaction to allergies need to have an emergency plan before their first day of care. Health Care Provider signature will be required.

Enrollment For Current Families

Enrollment renewal is not automatic!  Families who plan on returning will need to complete new, updated registration forms each April for the following year. Your child's immunization record is still on file and will be reviewed for any updates. For you, the school year registration fee and tuition is not due until the beginning of the school year in September. Summer tuition still requires a deposit from current families.

Enrollment For Pre-Kindergarteners          

Our Pre-K registration opens the first week of January for the following year (Wednesday, January 4 at 8:30 AM). We'll need registration forms, contract and one third of one month non-refundable deposit (we'll apply this your first month.) Immunization records are due anytime prior to the first day of school.

 

Registering for Full Days During the School Year

Excluding most legal holidays during the school year, we open for full days when the school is scheduled to close. Families needing care on full days will need to register three weeks in advance. A Registration Clipboard will be available on the front counter. Simply add your child's name to the list. A waitlist will be available when we reach capacity; adding your name to this will help us determine the need for additional staff. The Holiday Care fee will be added to your account. There is a two week cancelation policy when you register for full days.

Please Note...

  1. School Year, School Holiday Care and Summer Camp are separate registrations.
  2. Families needing full day child care on school closer days are able to sign up three weeks in advance.
  3. All registration is on a first-come basis and a wait list is created when we reach capacity.
